• Take photographs and/or video recordings of me for the sole purpose of advertising.

• Use these recordings, images, or audio in any format, including social media, websites, marketing materials, and advertising.
   

• Edit, alter, copy, exhibit, or distribute this media for any lawful purpose.
   

I understand and agree that:

• I will not receive any payment or compensation for the use of my image, video, or voice.
   

• The media may be used in various formats without notifying me. Although New Life Health Food, Inc. will use my image and/or video with good intentions, I understand and acknowledge that the company is not responsible for any unauthorized reposts, sharing, or misuse of the content by third parties once published.

• I understand that New Life Health Food, Inc. will do their best to stop any inappropriate or misuse of my video content.

• My participation is voluntary, and I may revoke this consent at any time by submitting a written request to nlhealthchicago@gmail.com, which will apply only to future use.
